Hi all..
I had a bad ICU call few days ago. It was horrible. I was just mega-exhausted. Our ICU is 22 bedded, and one person taking care of 22 beds during on call is mad. Really.. It’s the Intensive Care Unit.. not the normal ward. if one patient collapsed is still do-able. But there was one time, I was on call, and I had 3 patients collapsed and needed CPR at the same time!! MashaAllah.. The angel of death must be really busy that night in the ICU..
Anyway, back to my on call story. My ill patient that night was a dengue patient.
Dengue nowadays are so much different. It’s getting more and more difficult to manage. They need ICU care just for dengue. At the place where I work, we even have a dengue ward. The load of dengue patient in the hospital is getting worse over time.. and mortality or death due to dengue fever has increased significantly. Since January 1, 2014 until now.. there’s already 22 people died of dengue!!
Yes, it’s because of this mosquito called Aedes Aegypti

This Aedes mosquito is the carrier of flaviviruses. (Yes, dengue is a viral fever!), so, no antibiotic needed unless you have a secondary infection..
But seriously, all of us have to play part in helping to eradicate the mosquitos. When you use the mosquito spray, it only works on the adult mosquitos. But it doesn’t kill the mosquito eggs!! This is the part where you should ask me.. where does this mosquito eggs live?
The mosquito eggs live in stagnant water!! Yes, probably it’s in our flower vase, or the kolah air in our toilet, clogged drain, rooftop gutter or construction areas. Please, I know we can’t go and clean the construction area but at least what we can do at home is remove all stagnant water in our house / compound. Change the water in the flower vase everyday (yes, you heard me!!) Make sure the drain around your house is working well, no leaves or anything that can clog the water flow in the drain..
Aedes usually comes out twice a day, during dusk and dawn.. What I do at home, is to close all windows and doors early. And mosquito repellant to prevent mosquito bites. Sometimes the lotion repellant, sometimes the sticker repellant. For the toilet that we rarely use, I keep the toilet seat cover on, so the stagnant water in the toilet bowl won’t be the ground breed for this Aedes fella..
Typical symptoms of dengue is fever, bone pain (myalgia), joint pain (arthralgia), loss of appetite, common flu-like symptoms, headache and pain behind your eyes (retro-orbital pain).
The technology nowadays can even detect dengue at day one when you started to have fever. There’s a blood test called NS-1 to detect dengue virus in your body. Usually, during first of fever, your platelet count will not be so low. (Lucky there’s this NS-1 blood test that is very specific for dengue fever!). So, if you live in a prone-dengue area, and having dengue symptoms, get yourself check early.
Please, please, please..
Let’s do our part. Don’t let the mosquito breed easily. Enough people dying of this illness. Prevention is better than cure.

Salam Dr, Anak saya 3 tahun demam since saturday lagi..temperature sabtu hingga ahad dlm 38-39..mlm ahad baru nie sy ada pergi ke hospital ampang… kecemasan. Dr bg tau saya.. anak saya baru demam dua hari x boleh nak ambik darah untuk cek denggi. utk cek denggi demam mesti 3 hari ke atas. lagi pun x ada tanda-tanda denggi, ank saya cuma demam sahaja. balik dr hospital mlm tu..sampai ke pagi saya dan suami x tido, khaira muntah..lepas muntah temperature turun 37.8. Alhamdulillah pagi sampai ptg isnin tu..ank saya dh ok..tetapi mlm..dia demam balik..temperature naik 38 semula..lepas bg mkn ubt…dia punya suhu turun sedikit dan mlm tu badan dia mengigil kesejukan..kebetulan klu demam sy x bg dia pakai baju.jadi smlm siang dia ok balik..mlm dia jadi lagi..badan panas temperatur 37.8..kali ni x mengigil…
saya nak tanya Dr..
perlu ke saya bawa ank saya pergi hospital untuk ambik darah..??
dan adakah ini simptom denggi..??
sy bkn Dr, but sy work in a hospital lab, so adelah skit2 medical background. toddlers/babies/kids ni, kte kena extra careful. if tak puas hati dgn hospital tu, pegila klinik pakar kanak2. sometimes Dr yg jge ER tu just HO, yg mungkin kurang pengalaman (sorry sy bkn underestimate). but i would like to point out that, it is more than worth it to get a second opinion. u can always ask for this NS-1 blood test. just to be sure.ask questions to Dr on what to do if this/that happen (during consultation). sy harap ur girl sihat. amin.

Nauzubilahiminzaligh…its been sontimes my hubby didnt give me fresh flowers, so the vase is dry, sometimes my son saw died insects in the vase. Hope we can have covered drain or pipeline in earth. Pray it ends, InshaAllah, ameen.

doc.saya nak tanya…kalau ade org dari KKM yang sembur nyamuk dekat kawasan perumahan kita…kita patut ke tutup pintu rumah untuk elak asap tu masuk?saya selalu nak buka biar nyamuk dalam rumah mati..tapi mereka selalu suruh tutup pintu..mcm mane domtor
Hai Nak Tahu., hehe i kerja kesihatan. Please please please open ur door because masa dorg buat fogging, sometimes masa tuh jugak lah nyamuk lari masuk rumah and menyorok. tak pernah dengar lagi org KKM suro tutop segala pintu tingkap. yang disuruh tutup adalah makanan atau minuman., jgn biar terdedah. selepas fogging, kalau dlm rumah rasa berminyak, lap dulu dengan tuala kering then baru u mop pakai sabun or apa2 je lah yg sepatutnya. Sekiranya masa fogging u nak keluar, spray dlu pakai spray nyamuk espcialy tempat gelap n tersorok, langsir, celah baju, tuala. then close ur window n pintu.. 🙂
Assalamualaikum Dr,sy silent reader Dr selama ni,n mak sy pernah masuk icu d hos.selayang bed 11 rahmah bt mahamood, warded 11.12.13 n discharged 7.1.14 ..tanggal 10.1.14 mak sy meninggal dunia krn denggi slh satu sbbnya n Dr linda yg incharge slama mak sy d cni tp Dr pn slh sorang team yg mjaga mak sy,jutaan trima kasih pd team icu yg sgt komited dn cekap.

The virus is already mutated hence, it’s much lethal than how it used to be back then.
suhana…anak kedua saya jan13 mengalami demam 38°-39°..denggi test dibuat hari kelima ttp negatif..setelah 1 minggu tiada tanda2 positif & bertukar klinik..refer ke hospital.rupanya pnenoumonia 5% paru2 dijangkiti kuman..14 hari warded due to 2 antibiotik x beri kesan & kuman merebak 20%..alhamdulillah allah panjangkan umurnya..doc pakar decide antibiotik Tamilflu..the next day demam makin kurang..& terus hilang.

Pergilah buat futher assesment untuk anak suhana..dec13 anak 1 demam dan menunjukan simptom2 sama dgn adik.alhamdulillah pulih selepas 2 minggu setelah mendapatkan pelbagai pandangan dari beberapa pihak..bukan denggi hanya demam viral biasa..itupun bertukar.antibiotik…churam..dr halina…dulu2 kalau demam panadol dah boleh baikkan…by the way thanks for sharing the dengue info

Ingin dimaklumkan bahawa semburan fogging hanyalah membunuh nyamuk Aedes dewasa, namun sekiranya telur dan jentik-jentik masih ada dalam air bertakung, nyamuk Aedes dewasa masih boleh menetas. Justeru, fogging ini hanya penyelesaian sementara. Ketika penyemburan “fogging” KKM, pihak penduduk memang digalakkan membuka pintu untuk membenarkan pasukan penyembur masuk menyembur rumah, atau sekurang-kurangnya membuka pintu bagi membenarkan asap fogging masuk ke dalam rumah. Ini bagi memastikan semburan fogging luar tidak pula “menghalau” nyamuk Aedes ke dalam rumah. Dengan liputan dalam dan luar rumah, nyamuk dewasa Aedes dijamin akan mati.
Namun demikian,penduduk disarankan supaya menutup semua makanan bagi mengelakkan kontaminasi makanan oleh bahan kimia fogging. Pihak KKM juga sudah mula menggunakan semburan air dalam rumah bagi mengelakkan kesan berminyak dalam rumah yang pernah dikhuatiri penduduk.

anak saya pernah kena denggi bulan 7 tahun lepas time tu dia umur 7bulan..demam panas and warded kat hospital gov..tapi dia cakap darah tak de pape..so hari ke 3 saya mintak nak bawa anak saya keluar hospital sebab nak bawa dia pergi ke hospital swasta..bila kat swasta tros amik darah tak sampai sejam dah keluar result anak saya positif denggi…so saya sarankan la bawa anak2 yang demam panas ni pegi check darah kat hospital swasta..sbb hospital gov akan ambil masa… so keluar duit sikit untuk check darah kat hospital swasta..anak saya sampai kena masuk NICU sebab denggi…mula2 kena warded kat NICU kt hospital swasta tu..tp doc kanak2 kat hospital swasta tu ckp better dia transfer kan anak saya kat GH sebab kat GH ada bank darah…tapi ape yang penting kita tahu kenapa sebenarnya anak kita demam panas…so kt GH diorg dah tau ape sakit anak saya so just proceed ape yang patut.

Salam doc, anak sy kena demam denggi lebih kurang 11/2 aritu.. kena admitted for few days pastu ok dah…platlet dh naik sume dah.. cuma lepas discharged tu merah2 kt kaki n tgn tu rasa gatal2 saje… n symptom sakit perut tiba2 slalu dtg on n off.. selain dr itu dia sihat n aktif saje.. tp tiba2 ptg smlm 28/2 dia demam panas sgt n dia kata kaki n tgn dia lenguh2.. so arini gi jumpa doc n buat test darah n doc kata positive denggi..n platlet dia drop semula..tp not as bad as masa dia kena admitted aritu tu lah… doc suruh dtg lg 2 hari lg… so what say u doc? How bad is this reccurrence dengue?? Btw anak i umur 5 thn..
i can’t tell. maybe it’s still the same unresolved dengue. that is more common that another episode of dengue in few days apart..

Saya ingin bertanya. saya sekarang mengandung 6 bulan, dan start dr rabu lepas (26.02.2014) start demam. start pd hr tu juga, hari2 doc di hospital swasta minta saya datang utk test darah sehingga hr ini. (3.03.2014)… adakah ok utk dtg utk ambik darah shj ataupun, saya perlu masuk air utk mempercepatkan utk pulih? blh mendatangkan apa2 kesan pada kandungan saya x?

it’s ok to monitor blood results. is it dengue? intravenous drip (mask air) if you have poor hydration status. your doctor will decide for you..

My son (3 yrs old) has G6PD deficiency… Could u suggest any product suitable to be used in our house & on him. I usually bring him out to shopping mall when KKM is coming to our area to fogging…(Doctor mention that he cld not smells it)…
